Items Needed at your initial Assessment Session

  1. Intake Paperwork & Consent forms completed​ prior to your first session (forms will be sent to your email or the client portal app).

  2. Copy of front & back of clients insurance card (can be uploaded in the client portal app, texted or emailed picture to therapist).

  3. ADHD and/or Autism Diagnosis Paperwork uploaded into Client portal app or paper copies brought in to be scanned.

  4. IEP or 504 plans can be uploaded in the client portal app or paper copies brought in to be scanned.

  5. Debit or Credit Card to leave on file (self pay/private insurance), or inputted into the client portal app. 

  6. Throughout clients time in therapy, the client portal will be used for therapist to send client/caregiver treatment plans and documents to sign.
