Verifying Insurance Benefits and Eligibility

Clients are responsible for the copayment and deductible established by their insurance. 

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotheraapy services. You are responsible for all fees your insurance does not cover the same day of service. Sometimes we are unable to verify the exact amount a client will owe for therapy services until we recieve the EOB from the insurance company after the claim has been processed.

We recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health/behavioral health benefits for outpatient in office?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Do I have a co-payment or co-insurance? If so, how much is it? 

  • Will I have an out of pocket amount?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • If my therapist does not accept my insurance, what is the process for me to get reimbursed after I have paid for counseling services from an out-of-network provider?

  • Do I need written approval from my primary care physician in order for services to be covered?

In order for us to verify your insurance benefits we will need the following information:

  • A copy of the front and back of client's insurance card

  • Type of Insurance

  • Member ID

  • Policy Holder First & Last Name 

  • Policy Holder DOB

  • Policy Holder Address

  • Child (Client) First & Last Name 

  • Child (Client) DOB

  • Phone Number

For clients with other insurances we do not accept, we offer self-pay rates and superbills should you decide to seek reimbursement from your insurance provider. 

Clients with Primary and Secondary Insurance

In order for us to submit claims effectively for therapy services the following is required: 

  • We need to know which primary and secondary insurance the client has.

  • We need to know which insurance is primary and which is secondary for the client.

  • We need a picture copy of the primary and secondary insurance cards front and back texted to your therapist or

  • The policy insurance holder needs to call and inform both insurance companies (primary and secondary) that the client has another insurance. Sometimes this requires policy insurance holder to fax or email a copy of the insurance card, client DOB and member ID to the insurance companies to be placed on file.
